Select from the links below to print registration and procedure prep forms.  Please note we are not currently able to accept your paperwork through our website or via email.  If you have an office appointment, you should bring your completed paperwork to your appointment.  If you have a colonoscopy or other procedure scheduled, please fax your paperwork to (254) 537-0293 or mail it to us two weeks before your scheduled appointment.

Procedure Preparation Instructions

It is important that you follow your doctor's instructions carefully in preparing for your colonoscopy.  DO NOT follow any of the instructions below without your doctors instruction.  If you take any medications, be sure to contact our office 2 weeks prior to your scheduled procedure for specific instructions on taking your medications.
