Thank you for choosing our office for your digestive health care. To assist us with your care, please arrive 15 to 30 minutes early to complete your registration forms if you have not already printed and filled them out. Also please bring the following items to your first visit:
- Your insurance card and a photo ID
- Name and contact information for an emergency contact
- A list of medications with dosage that you are currently taking (or your medication bottles)
- A list of any known drug allergies
- Results of any related medical or laboratory tests
