By law, every person is entitled to copies of their medical records. This includes lab and test results and doctor's notes.
Release of Medical Records
We will be glad to provide copies of your medical records; however, depending on the amount of pages requested, you may be charged a fee. Most times, you only need one or two pages of your records, and not your complete medical history with our office. All requests for medical records must be submitted in writing. Please give us 3 to 5 days to complete your request.
Medical Records from a Referring Physician
When your doctor refers you to our practice, we typically request a copy of your medical records, including recent lab work and imaging tests. If your doctor does not provide this information to us by the time of your first visit, you may bring these documents with you.Click here for a printable Medical Records Release form.
